ALCOA+, one line each

Attributable — every entry traces to a named person. Legible — readable for the record’s whole life. Contemporaneous — recorded when the work happened. Original — the first capture, or a verified true copy. Accurate — reflecting what actually occurred. The plus: complete, consistent, enduring, available. None of these are exotic; all of them fail quietly in ordinary systems.

Attributable: the shared-login problem

One generic account at the instrument, one password on a sticky note, and every record in the system becomes anonymous. Inspectors test this in minutes: they ask who made an entry, then ask how you know. Unique accounts with role-based access are the floor, not the goal.

Contemporaneous: the Friday-pen pattern

Ten daily checks initialed in one sitting; completions timestamped seconds apart at shift end. Batch documentation reads as reconstruction, and reconstruction reads as fiction. The cure is workflow, not policy: records created at the bench, at the moment of the work, because the task arrives there.

Original and accurate: corrections that preserve

A correction is legitimate; an overwrite is not. The prior value, the correcting person, the reason, and the timestamp must all survive — 21 CFR 11.10(e) makes obscuring prior values a defect in itself. Systems built on append-only storage make the compliant behavior the only possible behavior.

The review obligation nobody staffs

FDA guidance expects routine audit trail review — and a trail nobody reviews eventually becomes the finding. Manual review of everything is unstaffable; exception-based review is not. A system that learns your operation’s baseline and surfaces the off-pattern — the correction cluster, the off-hours activity, the silence where activity should be — turns the obligation into a five-minute daily brief, with the review itself documented.
